Options for Community Living This program provides in-home assistance to elderly (age 60 or older) and disabled adults (age 18-59) that need assistance to remain safe and independent in their own homes. Services include periodic assessment, homemaker services, personal care services, and home-delivered meals. Service delivery is based on the individual’s need and the individual may be required to contribute toward the cost of homemaker or personal care. No cost shares are applied to meals. Call 1-931-490-5914 Homemaker Services - Older Americans Act Provides assistance to elderly persons with activities of daily living, such as: preparing meals, housework, and shopping. Call 1-866-836-6678 Public Guardianship/Conservator - Court Appointed Conservatorship services are provided directly by South Central TN Area Agency on Aging & Disability to aid persons 60 years of age and older who are unable to manage their own affairs and who have no family member, friend, etc. willing and able to act on their behalf. Call 1-931-490-5888 SHIP /SMP The State Health Insurance Assistance Program, or SHIP, is a nationwide program that provides free and objective counseling to persons with questions or problems about Medicare and related insurance products. This includes Medicaid, the Medicare Part D prescription program, Medicare Advantage plans, Medicare Supplemental Insurance and long term care insurance. The U.S. Administration on Aging (AoA) has changed the name of the program designated to help older Americans avoid unintended errors and detect deliberate abuses of the Medicare system. The program formerly known as the Senior Medicare Patrol Project (SMP) is now known as SMP: Empowering Seniors to Prevent Healthcare Fraud. If you have questions or are interested in learning more about these programs call toll free 1-877-801-0044 National Family Caregiver Support Program Unpaid family and friends are the main support for many elderly individuals that require help to prevent or delay nursing home placement. This program is designed to ease the stress on these caregivers and provide needed support to help them take care of their loved ones at home. Services include service coordination, caregiver counseling, support groups, respite care, caregiver training, and supplemental services such as home-delivered meals and a PERS (Personal Emergency Response System). Respite care may be authorized for up to 4 hours a week to give the caregiver temporary relief from care giving duties. A cost share may apply and is based on the care receiver’s income. Nutrition Services - Older Americans Act Congregate and home-delivered meals are provided at 21 different meal sites across the South Central District, along with Nutrition Education Programs and other supportive services. An Information Specialist answers this phone, provides information and referrals to help consumers connect with available resources and programs, and screens callers to see if they qualify for an in-home assessment to determine eligibility for in-homes services. Call 1-866-836-6678 Medicaid Waiver Enrollment in this program is based on the following requirements:· Resident of Tennessee · Financially determined by the Department of Human Services to be Medicaid eligible· Have an approved Pre-admission Evaluation (PAE) for level 1 nursing facility car Be an adult over age 21,be able to receive services at home at a cost less than that of institutional care, and have adequate social support. Legal Assistance - Legal Aid Society of Middle TN & The Cumberlands Provides legal advice and representation by an attorney, including legal casework and referral. They also provide legal education materials and programs to inform older persons of their rights, the legal system, and alternative courses of legal action. Call 1-800-238-1443
